Check Out Policy

AMSEA loans videos only to qualified AMSEA instructors. During the current budget cycle, AMSEA is able to waive rental fees through the agreed-upon check-out period. However, a rental fee of $1.00/day will be charged for books and videos returned after the due date. Extensions of due dates may be renegotiated depending on availability.

If you do not currently have an existing relationship with AMSEA, you will need to leave a credit card deposit with AMSEA at the time of your request. The deposit would cover late fees or replacement costs. Contact us by phone or fax with your credit card information. Please do not send credit card information electronically.

AMSEA'S LOAN TERMS - Please read the following terms and check the box below prior to submitting this form.

AMSEA will send items to you in good working condition. AMSEA's inventory gets a great deal of use throughout the year. In order to ensure its long life and availability to others in need, AMSEA requires you to agree to the following terms prior to borrowing any items.

By checking the box below, I commit to:

  • Request items with as much advance notice as possible, realizing that they are loaned as available.
  • Report any damaged or defective items to AMSEA as soon as possible, regardless of cause.
  • Being liable for replacement costs of any item/items, if I fail to return any items borrowed from AMSEA BY 30 days past date due.
  • Contact AMSEA to request an extension if the items are needed longer than expected, understanding that a late fee may be charged for unapproved late returns.
  • Being responsible for all return postage/shipping.
  • Return the items to AMSEA -- DRY and packaged as received -- within 24 hours after use or the first available opportunity for shipping.
  • INSURE the equipment with the return shipper (AMSEA will provide the value if needed).
  • Pay for return postage/shipping and insurance.
