Pre-course Reporting Form - Feel free to call AMSEA at 907-747-3287 or email the Course Coordinator with details about your class plans. The Pre-Course form is not necessary. It is important we know what classes our instructors are teaching, via phone, email, or pre-course report.
Course Roster - Required for Drill Conductor Courses.
Skills Checklist - Required for Drill Conductor Courses.
Student Sign-In Form - Required for Drill Conductor Courses.
Student Evaluation - Required for all courses.
Instructor Evaluation - Required for all courses.
Ergonomics Pre-Test - Required for Ergonomics Course
Ergonomics Post-Test - Required for Ergonomics Course
Course Fee Waivers - Required for all Drill Conductor classes receiving partial or full funding through grants or AMSEA funding. Please contact us if you have questions regarding funding for your individual Drill Conductor course.
Assumption of Risk Waivers - Required for all courses.
Post-course Reporting Form - Required for all courses EXCEPT DRILL CONDUCTOR COURSES.
NOTE: Please give as much advance notice as possible for Drill Conductor classes. DC class paperwork must include:
Course Fee Waivers (any class receiving partial or full funding from AMSEA or grants)
Assumption of Risk Waivers
Photo Identification for each person taking the class
Beating the Odds (PDF Version) - Email the Course Coordinator for an electronic copy.
